My nephew can't sleep through the night. Is something wrong?

My nephew spends the night on occasion and usually wakes up screaming and crying. He has episodes where he is recalling his day and we can't seem to wake him, even though his eyes are open. My sister in law has taken him to the doctor with no success of finding out what is going on. He is only 4 y/o. After watching Dumbo, his episode was because he couldn't fly. After visiting the ocean, he was screaming about the water. I feel really bad. It's more uncommon for him to make it through the night without an episode. Is something wrong?
