Diabetes Travel Letter: General


To Whom It May Concern:

_ (Date of Birth) ******_******, has Type 1, or insulin dependent, diabetes and is under the care of the Diabetes Team at Childrens National Medical Center, in Washington, D.C. He/she will be traveling, and will need to carry diabetes related supplies with them at all times. This will include the following items:

  • Insulin vials or cartridges
  • Insulin pen(s)
  • Insulin pump and infusion sets
  • Insulin syringes
  • Blood glucose meter(s)
  • Blood glucose test strips
  • Lancing device
  • Lancets
  • Glucagon Emergency kit(s)
  • Ketone test strips
  • Fast acting carbohydrates, such as glucose tablets or gel, juice boxes or regular soda
  • Extra snacks
