Most cases of prostate cancer do not have any symptoms so there are none to look out for. There are several PSA assays that are used. The free PSA refers to the circulating PSA that is not bound to any proteins. The majority of PSA is is bound to proteins. Free PSA tends to be lower in patients with prostate cancer, especially when it is less than 10%, and is most useful when the total PSA is between 4 and 10. The free/total ratio is used to help determine benign and malignant disease. Lower ratios are associated with a higher likelihood of prostate cancer. Your ratio of 13 is fine and not associated with malignancy as long as you have a normal digital rectal examination as well.
