When Is The Best Time To Take Daily Levemir If Blood Sugar Is Spiking 2 Hrs After Breakfast?

My father in law is 88 years old and has Diabetes 2. He was recently started on on Levemir Insulin on a daily basis,

He takes 10 units of Levemir at 9:00PM.

His FBS in the AM is 92.

He has 2 carb choices at breakfast and his blood sugar 2 hrs later is 237. The remainder of the day it never goes over 145.

What is the best time for him to take his Levemir insulin?

Thank you for your question. Does he take any other type of insulin with his meals, or does he take other oral medications for his diabetes? If the problem is occurring only after breakfast, and all his other daily readings are okay, then perhaps his breakfast diet needs to be adjusted, or a short-acting insulin could be added on to control his during the morning hours. Since you report that his fasting sugars in the morning are controlled, and his other sugars throughout the day are on target, the Levemir seems to be doing its job in fulfilling his basal insulin requirements. It would be helpful to the doctor if you keep track of his blood sugar readings and bring them to appointments. Then you can point out to the doctor that his readings are controlled most of the time, but are running consistently high after breakfast, and you can discuss how to gain control of those readings.
